Failed submission?

I am getting this under "my submissions". What am I doing wrong? I just uploaded the CSV file in the recommended format. Does the filename matter too? Cause my filename is output.csv:
Never mind. It worked after I changed the filename to sample_submission.csv and added 0 0 0 10 10 for images which my model didn't predict anything for.
filename doesnt matter but you do need to make a prediction for each and every image :)
